SHIPYARD PARK Lesson/Camp/Clinic/Training WAIVER

All guest should consult their physician prior to attending Shipyard Park for lessons, camps, clinics, and/or training.

I waive and release Shipyard Park, LLC, its staff, and hosting facilities from any and all injury that may occur during the use of the facilities, practices, instruction, and/or camps to the above participant or team members, onsite or involving travel to and from the event. I also take full responsibility for any expenses that occur during the use of the facility, lessons/practices, instruction, training, and/or camps/clinics due to necessary treatment of injury. I also give permission for emergency treatment if needed. I understand by signing this waiver, I give consent to the participation in this event and assume all risk.

Visitor agrees to hold Shipyard Park free from and indemnifies Shipyard and its owners, members, employees, officers and agents, against any injury, loss, cost or expense that may arise from or in connection with visiting, participating in activities, or training at Shipyard Park.
